Supervision

Under the supervision of the Home Care Agency Director RN and Center Administrator (PACE), the RN Care Manager assists with managing the flow of the clinic, leading all staff in the provision of medical and nursing care, ensuring high quality standards. Coordinates services with primary care team, acting as the primary liaison. Triages and assesses participants as needed. Serves as the primary medical contact person for the Care Management Team along with the MSW and Provider. Provides skilled nursing care to participants, implements orders, develops appropriate plans of care, provides participant, caregiver, and family teaching, and communicates participants’ condition to caregivers, primary care, and the Interdisciplinary Team. Shares the responsibility for overseeing the well-being of the participants with a MSW and a Provider. Facilitates the initial assessment, assessment, and re-assessment, coordination, monitoring, and provision of health care services and treatments for program participants. Assists in facilitating the integration of new participants into the health care delivery system, including, but not limited to, the oversight and management of medications, immunizations, routine monitoring of chronic problems, and nursing care plan development. Participates in utilization review, quality improvement initiatives, and ensures compliance with policies and regulations.

Minimal Education:
Graduate of accredited school of nursing.
Bachelor’s degree of Science in Nursing is preferred.

Minimal Experience:
Minimum of two years’ clinical nursing experience.
One (1) year home care, long term care setting, care management, or PACE experience preferred.
A minimum of one year’s experience caring for the frail and elderly.

Other Qualifications:
Current RN Nursing License.
Cardio Pulmonary Resuscitation (CPR) certification and First Aid required.
Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Must have a valid driver’s license, proof of insurance, and have means of transportation. Must be able to meet auto insurance carrier’s standards for coverage.
Ability to meet specific competencies for the Registered Nurse prior to providing participant care.
Ability to function with minimal supervision.
Must have strong interpersonal, organizational, group facilitation, and consensus-building skills.
