Position Summary

The Client Care Coordinator plays a vital role in delivering exceptional client care and support to our clients and their families. This position requires strong communication, organization, and relationship-building skills, as well as the ability to collaborate effectively with internal team members to ensure seamless care. This is a full-time position, Monday through Friday, from 8:00 AM to 5:00 PM, with a one-hour lunch break each day.

Key Responsibilities

  • Coordinate and schedule client appointments and in-home visits
  • Maintain accurate, timely documentation of client information and care plans
  • Respond to client and family inquiries professionally and promptly
  • Collaborate with internal care teams to support high-quality, coordinated client care
  • Assist in developing and implementing effective client care strategies

Qualifications

  • High school diploma or equivalent required
  • Previous experience in client care, healthcare, or a related field preferred
  • Excellent communication and interpersonal skills
  • Strong organizational, time management, and multitasking abilities
  • Ability to work independently and as part of a collaborative team
  • Must be able to travel in and around all of the Central Oregon area

Compensation & Benefits

  • Hourly wage of $25, paid bi-weekly
  • $0.50/hr increase for bilingual in Spanish
