Pay Rate: $55–$58 per hour

Position Summary

"Navitas Healthcare, LLC" is seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to provide comprehensive social work services to patients and their families. This role plays a critical part in care coordination, psychosocial assessment, and discharge planning while working collaboratively with interdisciplinary healthcare teams to support optimal patient outcomes.

Key Responsibilities

• Conduct psychosocial assessments and develop individualized care plans

• Provide counseling, advocacy, and crisis intervention services

• Coordinate community resources and facilitate referrals as needed

• Assist patients and families in navigating healthcare systems and social services

• Ensure accurate, timely, and compliant clinical documentation

• Collaborate closely with physicians, nursing staff, and interdisciplinary team members

Required Qualifications

• Minimum of 1 year of experience as a Licensed Master Social Worker (LMSW)

Education

• Master’s Degree in Social Work (MSW) from an accredited institution

Licensure & Certification

• Active Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED)

• Primary Source Verification of licensure required
