Social Worker Assistant

Job Overview:
Our center has been serving the elderly adults with chronic, medical & cognitive or mental health condition. We are looking for Social Worker Assistant, to join our team in working towards the company goal of promoting and restoring participant 's health.

Responsibilities:
- Assist with processing admission and enrollment of participants.
- Record attendance and complete daily flowsheets.
- Maintain records of participant progress notes.

- Maintain confidentiality of participant records according to HIPAA regulations.
- Educate families and participants in regards to health and prevention of health decline.

Requirements:
- Bachelor's Degree in Social Work or related fields
Preferred Languages : English, Farsi

Job Type : Full Time

Pay: $20 per hour

Schedule :

- Day Shift

- Monday to Friday
