Based at River Grove, you will be the trusted bridge connecting patients, families, healthcare providers, and our facilities. In this role, you’ll guide families through the admission process, advocate for patients, and ensure a seamless transition of care while building meaningful relationships across both locations.

Essential Duties and Responsibilities of a Community Liaison:
Conduct thorough pre-admission assessments, gathering clinical and financial information to ensure timely and appropriate responses to referrals.
Collaborate closely with discharge planners, hospital case managers, physicians, and managed care organizations to prioritize referrals and coordinate transitions of care.
Strengthen external partnerships by fostering ongoing, meaningful relationships with hospital-based teams, expanding awareness of River Grove’s clinical capabilities.
Educate hospital staff— including ER physicians—on skilled nursing services, specialty programs, and the types of patients who can safely transition to River Grove.
Serve as a compassionate resource for patients and their families, answering questions, explaining services, and helping them feel supported prior to admission.
Identify special clinical needs (equipment, medication, specialty services) to ensure our facility can meet patient requirements before admission.
Collaborate with nurse managers to support staff training needs for higher-risk admissions.
Visit patients and families bedside to conduct assessments, discuss care needs, and initiate appropriate care plans.
Maintain full knowledge of and compliance with HIPAA and all federal/state regulations regarding confidentiality and documentation.
Display professionalism, positivity, and clear communication in every interaction—building trust with families, partners, and internal teams.
Verify insurance eligibility, review documentation for medical necessity, and obtain pre-authorizations for admission.
Submit appeals for denied requests and coordinate with providers as needed.
Act as a liaison to help patients understand their financial responsibilities related to care.
Identify new growth opportunities by promoting services, building community partnerships, and increasing awareness of River Grove’s strengths.
Represent the facility at community events, speaking engagements, and outreach programs.
Collaborate with the facility marketing team to enhance River Grove’s visibility and highlight core competitive advantages.
Required Knowledge, Skills and Abilities:
Exceptional verbal and written communication skills.
Strong interpersonal abilities with a customer-service mindset.
Prior experience in healthcare—especially acute care or skilled nursing.
Proficiency with computers, databases, electronic health records, and medical terminology/coding.
Strong organizational skills with the ability to multitask effectively.
Bachelor’s degree or equivalent experience preferred.
Clinical/nursing background is a plus—especially experience in discharge planning, case management, or utilization review.
Familiarity with local hospitals, physicians, and referral sources strongly preferred.
