Join Penn Medicine as a Home Health Social Worker – Lancaster, PA

We’re seeking a compassionate Social Worker to support patients and families receiving home health care services. In this role, you’ll provide individualized care planning, assessment, intervention, and evaluation in collaboration with an interdisciplinary team.

Position Details:

• Location: Lancaster, PA
• Schedule: Full-time, Monday–Friday, 8 AM–5 PM

What You’ll Do:

• Assist patients and families with resources and care transitions, including hospice when appropriate
• Provide guidance on advance directives and emotional support during times of grief and loss
• Communicate empathetically and maintain confidentiality
• Assess psychosocial needs and identify patients at social or psychological risk
