We are seeking a compassionate and dedicated Hospice Social Worker to join our interdisciplinary care team. Responsibilities
HOSPICE SOCIAL WORKER EXPERIENCE -WILL DISCUSS PAY ON INTERVIEW
- Conduct thorough patient assessments to evaluate emotional, social, and psychological needs within hospice care settings.
- Provide individual, family, and group counseling sessions utilizing clinical counseling techniques such as motivational interviewing and behavioral therapy.
- Develop personalized care plans that incorporate mental health counseling, crisis intervention, and behavioral health strategies tailored to each patient’s condition.
- Coordinate with healthcare teams to ensure seamless integration of social work services with medical treatment plans, including documentation in medical records and EMR (Electronic Medical Records) systems.
- Assist patients and families with navigating complex healthcare systems, including hospice services, assisted living options, and community resources.
- Support patients with specialized needs such as dementia care, Alzheimer's care, PTSD care, autism spectrum disorders, or developmental disabilities through targeted interventions.
- Maintain compliance with HIPAA regulations by securely managing medical documentation and patient records while ensuring confidentiality at all times.
Skills
- Extensive experience in hospice care, mental health counseling, or behavioral health settings.
- Proficiency in child & family counseling, crisis management, and working with diverse populations including seniors and individuals with disabilities.
- Strong knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and clinical research methodologies relevant to hospice services.
- Skilled in intake procedures, diagnostic evaluation, case management, and working with EMR systems for accurate data collection and documentation.
- Ability to perform comprehensive patient assessments using diagnostic tools for mental health conditions such as depression or PTSD.
- Experience with home & community care environments including inpatient facilities, nursing homes, assisted living communities, outpatient clinics, and pediatric settings.
- Excellent communication skills for conducting individual and group therapy sessions; adept at motivational interviewing and cognitive behavioral therapy techniques. Pay: From $35.00 per hour
