Job Summary
We are seeking a dedicated and compassionate Home Health Master Social Worker to join our dynamic healthcare team. Duties
- Conduct thorough patient assessments to evaluate social, emotional, and environmental factors impacting health and recovery.
- Develop personalized care plans that incorporate behavioral health strategies, crisis intervention techniques, and social support systems.
- Provide individual, family, and group counseling sessions focused on mental health counseling, behavioral therapy, addiction counseling, and clinical counseling.
- Coordinate with healthcare providers to ensure compliance with HIPAA regulations and accurate medical documentation within electronic medical record (EMR) systems.
- Assist patients with navigating community resources, assisted living options, hospice care services, and specialized programs such as Alzheimer’s or dementia care.
- Facilitate diagnostic evaluations and case management activities for diverse populations including pediatrics, geriatrics, individuals with developmental disabilities or autism spectrum disorder.
- Engage in research activities related to behavioral health interventions or post-acute care practices to enhance service quality.
- Conduct intake assessments for new patients and manage ongoing patient care through continuous monitoring and documentation of progress.
- Utilize motivational interviewing techniques to promote patient engagement and adherence to treatment plans.
- Collaborate with interdisciplinary teams during inpatient or outpatient settings to optimize patient outcomes across various healthcare environments.
Experience
- Proven experience in hospital settings, home & community care environments, or outpatient clinics providing social work services.
- Demonstrated expertise in child & family counseling, behavioral therapy, crisis management, and mental health counseling.
- Familiarity with medical terminology, ICD-10/ICD-9 coding systems, CPT coding for billing purposes, and medical records management.
- Experience working with people with disabilities—including developmental disabilities—and providing specialized dementia or Alzheimer’s care.
- Strong background in working with diverse populations such as seniors in assisted living or nursing homes, children in special education programs, or individuals requiring PTSD or addiction treatment.
- Knowledge of EMR systems for data collection and documentation accuracy across clinical workflows.
- Ability to perform diagnostic evaluations using ICD coding standards while adhering to HIPAA regulations for patient privacy.
- Experience conducting group therapy sessions or psychotherapy interventions tailored to individual needs within outpatient or inpatient settings.
- Skilled in crisis intervention strategies with a focus on behavioral health emergencies across various healthcare environments.
