Position Overview

The Clinical Care Manager provides clinical and administrative support in a telephonic healthcare setting while coordinating patient care to ensure seamless delivery of medical services. This role involves assisting healthcare providers, managing patient records, and facilitating care plans remotely to improve patient outcomes, with all duties performed via telephone or virtual platforms.

Key Responsibilities

Clinical Duties (Telephonic)

  • Assist physicians and nurses by gathering patient information, such as vital signs or medical histories, through telephonic interviews.
  • Provide telephonic support to patients, explaining procedures, treatment plans, or medication instructions to ensure understanding and comfort.
  • Guide patients remotely in self-administering treatments or monitoring health metrics (e.g., blood pressure, glucose levels) as directed by healthcare providers.
  • Coordinate telephonic collection of patient data for basic laboratory test results or follow-up assessments, collaborating with external labs or providers as needed.

Care Management (Telephonic)

  • Coordinate patient care plans remotely, including scheduling telephonic follow-up appointments, referrals, and diagnostic tests.
  • Communicate with patients via telephone to provide education on treatment plans, medications, and lifestyle modifications.
  • Monitor patient progress telephonically, following up on care plans to ensure adherence and address barriers to care.
  • Collaborate with healthcare teams, including physicians, nurses, and specialists, via virtual platforms to develop and implement individualized care plans.
  • Maintain accurate and up-to-date patient records in electronic health record (EHR) systems, ensuring compliance with HIPAA regulations.
  • Assist with chronic care management telephonically, supporting patients with conditions such as diabetes, hypertension, or heart disease through regular check-ins and education.

Administrative Duties (Telephonic)

  • Schedule patient telephonic appointments and manage provider calendars using virtual scheduling systems.
  • Handle patient inquiries via telephone or secure messaging platforms, providing clear and professional communication.
  • Process insurance forms, billing, and coding for services rendered, managed through telephonic or online systems.
  • Track inventory of medical supplies for patients (e.g., home monitoring devices) and coordinate telephonic orders as needed.

Qualifications

  • Education: Experience with telephonic clinical support of patients and families; Preferred completion of an accredited Medical Assistant program
  • Experience: At least 1-2 years of experience in a clinical or telephonic healthcare setting; experience in care management or patient coordination is a plus.
  • Skills:
  • Strong interpersonal and telephonic communication skills to interact with patients and healthcare teams remotely.
  • Proficiency in EHR systems, telehealth platforms, and basic computer skills (e.g., Microsoft Office, virtual meeting software).
  • Knowledge of medical terminology, clinical procedures, and care coordination principles.
  • Ability to multitask, prioritize, and work efficiently in a remote, telephonic environment.
  • Certifications: additional certifications in care management or chronic disease management are a plus.
  • Attributes: Compassionate, detail-oriented, and committed to patient-centered care in a telephonic setting.

Work Environment

  • Full-time administrative office-based position, typically 40 hours per week
  • Work is performed telephonically
  • No in-person patient interaction or physical lifting of equipment is required.
