Position: Case Manager/Social Worker (MSW)- Discharge Planner
Location: Hummelstown,PA
Schedule: Fulltime
Compensation: starting at $70,000

Pay based on years of experience.

• The primary job functions in Case Management include:
• Clinical Interventions/Discharge Planning
• Care Planning Management
• Fiscal Management
• Payer/Referral Management

Minimum Qualifications

• Current Licensure in a clinical discipline per state guidelines required. (Registered Nurse or Social Work (SW) preferred)

Preferred Qualifications

• Previous experience in Case Management and Discharge Planning preferred.
• CCM Certification Preferred.
