LMSW,LCSW,LGPG,LCPC -Mobile Treatment Therapist

We are seeking a full-time Social Worker to provide individual therapy and case management for individuals with persistent mental illness, primarily on the Mobile (ACT) service.

• Serve as the primary therapist and treatment coordinator for a caseload of mentally ill patients.
• Deliver individual, group, and family therapy as needed.
• Assist in medication monitoring under psychiatrist supervision.
• Develop and implement treatment plans.
• Educate patients and families about services and community resources.
• Provide community outreach and in-home services.
• Offer case management support tailored to patient needs.
