Social Worker LMSW- Full Time Temp

$ 42.4792 to $ 52.2243 per hour, depending on actual experience

The Social Services Department in Manhattan Division of The New Jewish Home is seeking for a full time temporary Social Worker to join their team! The duration of work is between 4 to 5 months and maybe extended if needed.

Job Responsibilities:

• Completing MDS and biopsychosocial assessments.
• Facilitate care plan meetings & discharge planning.
• Implement Advanced Directives and End of Life Planning.
• Completing all required documentation in the medical record ( EMR)
• Coordination of care and services including ongoing monitoring of an appropriate and effective person-centered care plan; Strong Clinical, Communication and Organizational Skills

Qualifications:

• Demonstrates judgment of safety and confidentiality issues
• Strong verbal and written communication skills
• Strong customer service focus required
• Knowledge of the community resources
• Social work Long term care experience preferred.
• Master Degree in Social Work, licensed (LMSW)
