We are seeking a compassionate and experienced Social Worker to join our team. preferably Master's in Social Work (MSW), or a master's in related field (Gerontology, Psychology, MFT )
If you have a MA in a related field, you must have minimum one year of experience providing social services in the field of aging, health or long-term care.
**BA applicants are welcome to apply as long as they have minimum two years of experience providing social services in one of the fields of aging, health, or long-term care services.** MUST BE A BACHELORS IN SOCIAL WORK (BA in Psychology, sociology will not be accepted).
Duties: (include but not limited to) Complete intakes, facility tours, initial evaluations, reassessments, quarterly assessments, home assessments/visits, and flowsheets. Provide weekly support group counseling, case management, and discharge planning. Work closely with the LCSW providing counseling and to participants with mental health needs and more.
Required Language: Fluent in English/Mandarin
ADHC experience preferred
M-F / 30 hours week
Job Types: Full-time, Part-time
Pay: $27.00 - $28.00 per hour
Expected hours: 30 per week
Work Location: In person
