Job Summary

Under the general supervision of the Respiratory Care Supervisor and Manager, the Discharge Respiratory Therapist is responsible for the assessment, diagnostic and therapeutic interventions affecting the cardiopulmonary system for both children and adults. Selects and administers the appropriate care, taking into consideration the age, development level and size of the patient. Provides a caring relationship that facilitates health and healing. Identifies own limitations and seeks appropriate resources, as needed. The Respiratory Therapy Discharge Planner has well developed knowledge and skills in the assessment and acute care management of patients and families with respiratory disease. The scope of practice includes patient/family assessment resource management, care facilitation, and discharge planning of appropriate respiratory treatments including supplemental oxygen, positive

airway pressure devices, nebulizers, inhalers, mechanical ventilation, and other necessary treatment devices. The Respiratory Therapy Discharge Planner collaborates with the Nurse Case Manager and the entire care team in the development of care standards and guidelines for the purpose of improving quality of care, collaborating discharge, and reducing re-admissions. The RespiratoryTherapy Discharge Planner possesses a high degree of skill in clinical practice participating in direct patient care offering clinical expertise in assessment, recommendations, & implementation of care plans/equipment. The incumbent will collaborate with care providers and respiratory leadership to ensure quality and evidence-based practices that optimize patient outcomes.

  • Ongoing assessment and recommendations to address patients' needs, developing individualized discharge plans, and coordinating post-hospital care to ensure a smooth transition back home or other care facilities.
  • Developing comprehensive discharge plans in collaboration with the healthcare team, patients, and families, ensuring that all necessary resources and support are in place for a successful transition.
  • Works with the interdisciplinary team, including physicians, nurses, social workers, and case managers, to ensure a coordinated and cohesive approach to patient care.
  • Works with DME companies to ensure/arrange for necessary respiratory equipment is available and families are properly instructed on their use.
  • Seeks consultation from appropriate disciplines/departments as required to expedite care and facilitate timely discharges.
  • Provides accurate documentation and billing in the electronic medical record.

Minimum Qualifications

Associate's Degree

2 Years of Experience

BLS for Health Providers (AHA)

NRP Advanced Provider (AAP)

Ped Adv Life Support Prov(AHA)

Registered Resp Therapist

Respiratory Care Practitioner

In lieu of an associate's degree, a diploma from a NBRC certified Respiratory Therapist Program will be accepted for employees hired before 10/1/2013

Ability to take care of patients or patient equipment in the area in which you work, including the ability to handle patients with an airborne-transmitted disease. Must be able to comply with infection control standards that may evolve over time, including the routine use of PPE, required FIT testing and/or specialty training.

Asthma Education Certificate (AE-C) or must obtain within one year.

The current salary range for this position is $44.52 to $61.95
