This role supports a family primary care practice, triaging and connecting patients to behavioral health resources.
Responsibilities:
- Conduct patient evaluations
- Recommend treatment options and make referrals
- Educate and guide patients through available resources
Qualifications:
- Master’s in Social Work (LICSW, LMHC or LCSW )
- 3–5 years of direct clinical experience
- Experience with serious mental illness
Job Type: Full-time
Pay: $76,245.52 - $105,000.00 per year
