Definition and Distinguishing Characteristics
DEFINITION
Under direction, provide highly skilled, specialized and professional psychiatric treatment, counseling, therapy and case management services for Behavioral Health clients with mental and emotional illness, substance use disorder, and their families and support networks. Psychiatric services include diagnostic evaluations, ongoing assessments, recommending treatments, being part of a multi-disciplinary team, promoting health and wellness, 24/7 crisis intervention services when necessary - including mobile crisis, managing side effects of medications, and prescribing psychiatric medications; and to do related work as required.

DISTINGUISHING CHARACTERISTICS
This is a licensed Psychiatric Nurse Practitioner position, in the Behavioral Health Department. Incumbents are fully trained and competent to perform difficult, technical and complex duties requiring considerable professional knowledge and judgment. Work performed in this position, will be supervised by a staff Psychiatrist.

Behavioral Health Psychiatrist
CLASSIFICATIONS SUPERVISED
May provide lead direction and coordination for a variety of program staff, as assigned.

Examples of Duties
The following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Each individual in the classification does not necessarily perform all the duties listed.

  • Furnishes psychotropic medications under the supervision of a staff Psychiatrist and in accordance with standardized procedures; monitors individual's response(s) to medication; and educates individuals and members of support system.
  • Under the supervision of a psychiatrist, performs comprehensive psychiatric and physical evaluations including mental health status examinations, assesses current and past history of violence, suicidal, or self-harm behavior, substance use, level of functioning, health behaviors, trauma, sexual behaviors, multigenerational family assessment and social and developmental history to identify the nature of the individual's illness and establish diagnosis.
  • Provide professional psychiatric services for children, adolescents, transition-aged youth, adults and older adults including crisis intervention, for Amador County Behavioral Health clients.
  • Apply psychiatric principles in counseling clients, and assist them in making personal and social adjustments
  • Provide multimodal, trauma-informed, and culturally appropriate treatments and treatment recommendations for clients under their care or whom they are providing consultation.
  • Provide medication management, including Medication Assisted Treatment for Substance Use Disorders as needed.
  • Collaborates with multidisciplinary teams to deliver coordinated patient care. May consult and collaborate with other health or behavioral health professionals in the community.
  • Provide expert court testimony and/or reports to the court.
  • Maintain accurate and timely documentation of professional activities to high professional standards, and provide reports or summaries of these activities where indicated.
  • Maintain electronic health records of diagnosis, treatment and prognosis according to ACBH standards and other regulatory agencies.
  • May provide training and chart auditing for other staff, including quality assurance peer review functions.


Essential Qualifications
Knowledge of:
  • Laws and regulations governing Nurse Practitioners.
  • Laws, regulations and ethical standards governing behavioral health treatment and medical records.
  • Medicare and Medi-cal regulations.
  • Principles and practices of modern clinical psychiatry and general medicine, including differential diagnosis and treatment approaches
  • Scope of responsibilities applicable under incumbent's licensure.
  • Reporting laws on domestic violence, child and elder abuse, and neglect.
  • Medical laws and regulations, laws regarding informed consent and the ethics of patient care.
  • Medical aspects of psychological problems; psychosocial behaviors in illness.
  • Psychopharmacological use and effects of medications including medications and drugs commonly prescribed, including medications therapeutic effect, side effects, reactions and contraindications.
  • Principles and methods of psychiatric research and diagnosis.
  • Principles of mental health education and counseling.
  • Principles and practices of behavioral health education, community organizations and behavioral health services and resources available in the local area.
  • Substance use/chemical dependency and its relationship to psychiatry
  • Recovery-oriented principles of psychiatric care.


Ability to:

  • Provide a variety of psychiatric diagnostic services, therapeutic counseling, and psychotherapy for Behavioral Health/Medi-Cal clients.
  • Perform skilled evaluation and treatment of mental illness and substance use disorder.
  • Develop and/or consult on the development of effective patient treatment plans.
  • Apply psychiatric techniques in a community behavioral health setting.
  • Establish and maintain a good objective working relationship with clients in determining their needs and performing psychiatric diagnosis.
  • Communicate effectively both orally and in writing.
  • Communicate effectively with people from diverse ethnic and socioeconomic backgrounds.
  • Explain complex and technical terminology and concepts in an understandable and non-threatening manner.
  • Effectively represent the Behavioral Health Department in contacts with clients, the public, community organizations, other County staff, and other government agencies.
  • Regularly work well under pressure, meeting multiple and sometimes conflicting
  • deadlines.
  • Constantly demonstrate cooperative behavior with colleagues, supervisors, and customers/clients
  • Provide oversight/responsibility for dispensing pharmacy critical samples.


TYPICAL PHYSICAL REQUIREMENTS

Ability to sit for extended periods; frequently stand and walk; normal manual dexterity and eye hand coordination; lift and move objects weighing up to 25 pounds; corrected hearing and vision to normal range; verbal communication; use of office equipment, including computer, telephone, calculator, copiers, and FAX.
TYPICAL WORKING CONDITIONS
Positions in this class require the incumbents to:

  • Work is usually performed in an office environment, hospital, jail or other community based settings.
  • Employees may interact with members of the public or with staff under emotionally stressful conditions while interpreting and enforcing departmental policies and procedures.
  • Work during non-routine hours, including evenings, holidays and weekends.
  • Work outside in all types of climatic conditions including inclement, very hot or very cold weather.
  • Employees may work outdoors and are occasionally exposed to loud noise levels.

Training and Experience

Any combination of training which would likely provide the required knowledge and experience is qualifying. A typical way to obtain the required knowledge and abilities would be:
Education

Successful completion of an approved Nurse Practitioner training program in the required specialty area.
Possession of a valid California licensure as a Registered Nurse, Nurse Practitioner, certification to practice Psychiatric Services and a valid DEA number and certification.
SPECIAL REQUIREMENTS

Possession of, or ability to obtain, an appropriate California Driver's License issued by the Department of Motor Vehicles.
