The Clinical Social Worker - Discharge and Community Integration Specialist (Health - Correctional Facility - Safety) is under the supervision of the hospital Supervising Psychiatric Social Worker, and in consultation with the statewide DSH Program Director for Discharge and Community Integration (DCI) in the Social Work Services Department at the Department of State Hospitals - Atascadero (ASH). The incumbent will be responsible for the following duties but not limited to:
- Identify local barriers to implementation and collaborate to develop solutions.
- Coordinate with DCI Specialists at other DSH facilities, in collaboration with DCI Program Director.
- Identify community resources and develop cooperative partnerships.
- Develop and strengthen relations with outside agencies that provide housing, wrap-around care, or other services needed by discharging patients.
- Improve local understanding of discharge best practices.
- Support hospital implementation and program expansion.

1. BE LIST ELIGIBLE: Successfully complete the competitive State civil service exam pertaining to this classification at Clinical Social Worker (H-CF-S) Exam Bulletin, OR
2. BE TRANSFER ELIGIBLE: Have current employment with the State of California in the same classification or in a classification that is comparable (employees who wish to be considered based on transfer eligibility must meet the minimum qualifications of the classification per DPA Rule 250), OR
3. BE REINSTATEMENT ELIGIBLE: Have obtained previous permanent employment with the State of California in the same classification or in a classification that is comparable.

Working Conditions
This position requires you to work within the Secured Treatment Area (STA).
This position is classified as hybrid. It has options of the following schedules:
- Two 8-hour shifts in-person, one 4-hour shift telework per week
- Two 10-hour shifts in-person per week
