The Health Partner II is responsible for teaching patients about behaviors that promote health and wellness. The Health Partner II will also help the health care team develop and maintain positive relationships with patients by providing ongoing support for the patient and their families through the team based care model.
Responsibilities:
- Provide pre- and post- care instructions to patients according to TrueCare Policies and Procedures and in direct correlation to provider orders and a defined plan of care.
- Work in collaboration with all members of the health care team to determine appropriate plan of care for patients based on their individual care needs.
- Participate in daily team huddles to manage patient flow during appointment times.
- Interview and evaluate patients for individual educational needs, as determined by the care team, to support chronic care issues as well as complicated acute medical concerns.
- Provide educational support to the Health Partner I in managing patient care.
- Review care plans/self-management plans with patients to ensure the plan of care is being carried out (e.g., for a diabetic, labs and eye and foot exams are completed within appropriate intervals).
- Lead group educational classes for patients on topics designated by the health care team.
- Initiate and/or collaborate in policy and procedure development related to patient and staff education.
- Participate in case conferences, as directed.
- Participate in population health initiatives, as directed.
- Participate in periodic meetings and educational updates, as appropriate.
Qualifications:
- High school diploma or equivalent.
- Graduate of an accredited Medical Assistant Program.
- Successful completion of a Team Based Care Cohort.
- Two(2) years’ experience as a Medical Assistant, with at least one (1)year of experience providing health education to patients.
- Knowledge of medical terminology, medical laboratory procedures, normal human anatomy and physiology, chronic health care conditions and healthcare operations and work flow.
- Demonstrated ability to perform motivational interviews.
- Computer proficiency, particularly with Electronic Health/Medical Record systems and the Microsoft Office suite, including Outlook and Word.
- Associate's Degree in Healthcare related courses.
- Bilingual in English and Spanish (may be required at some clinics).
- Health Coach Certification or equivalent experience providing patient education.
