- Visit patients and assess their social, emotional, and practical needs
- Provide short-term counseling, crisis intervention, and support for patients and caregivers
- Connect patients with community resources (housing, food, transportation, financial support, etc.)
- Collaborate with the care team to create personalized care plans
- Support patients and families through serious illness, disability, or life transitions
- Keep clear, timely documentation using Home Care Home Base (HCHB)
- Who We're Looking For
- Master's degree in social work (MSW) required
- Licensed to practice in [Insert State] (LCSW preferred, but not required)
- Experience in home health or healthcare is a big plus
- Great listener, team player, and problem-solver
- Comfortable working independently and traveling to patients' homes
- Familiar with Home Care Home Base (HCHB) or willing to learn
