Office Care Coordinator

Overview
We are seeking a dedicated and detail-oriented Care Coordinator to join our team. In this role, you will serve as a vital link between patients and care providers, ensuring that patients receive the necessary care and support throughout their journey. The ideal candidate will possess strong organizational skills and a solid understanding of medical terminology, as well as experience with Customer Service.

Duties

• Coordinate patient care by scheduling visits, managing follow-ups, and ensuring timely communication between patients and care providers.
• Maintain accurate patient records and documentation, ensuring compliance with privacy regulations.
• Assist patients in understanding their care plans, and any necessary follow-up care.
• Provide administrative support within the office, including answering phones, managing correspondence, and handling inquiries.
• Collaborate with team to streamline processes and improve patient outcomes.
• Utilize medical terminology effectively to communicate with both patients and healthcare professionals.

Experience

• Previous experience in a medical administrative support role is preferred.
• Familiarity with In-Home Care office operations is a plus.
• Strong knowledge of medical terminology to facilitate effective communication.
• Excellent organizational skills with the ability to manage multiple tasks simultaneously.
• Proficient in office software applications and electronic health record systems.
