Job Summary
We’re seeking a qualified and compassionate Social Worker to join our interdisciplinary team. The ideal candidate will provide psychosocial assessments, counseling, and care coordination to participants while maintaining compliance with state regulations.
Key Responsibilities
- Conduct initial and periodic psychosocial assessments of participants.
- Develop and implement individualized care plans with the interdisciplinary team.
- Provide supportive counseling to participants and their families.
- Maintain accurate documentation and progress notes per Title 22 and facility policy.
- Participate in case conferences, discharge planning, and referrals to community resources.
- Promote participant independence, safety, and well-being through ongoing support.
Qualifications
- Master’s Degree in Social Work (MSW) OR BSW from an accredited program.
- Current California BBS registration (ASW or LCSW preferred).
- Minimum 1 year of experience with elderly or disabled adults (healthcare or community setting).
- Strong understanding of Title 22 ADHC regulations and interdisciplinary teamwork.
- Excellent communication and documentation skills.

Pay: $22.00 - $24.50 per hour
Expected hours: 25 per week
