Position Summary

The HCC Social Worker works with the HCC interdisciplinary team to assist in meeting the psycho-social needs of the residents, improving quality of life and service, and maintaining compliance with federal and state regulations. This position is integral to Minimum Data Set (MDS) development, Pre-Admission Screenings and Resident Reviews (PASRRs), care planning, completing required assessments/forms, and discharge planning (e.g. Notices of Medicare Non-Coverage (NOMNCs), assisting with placement, arranging services, etc.). This position promotes the mission, vision and values of The Redwoods and abides by its policies and procedures.

Primary Responsibilities

Social Service Functions:

  • Plan, develop, organize, implement, evaluate, and assume the authority, responsibility and accountability of directing the social service function in the Health Care Center.
  • Communicate with families and other responsible parties.
  • Coordinate supportive medical and non-medical appointments such as podiatry, beauty/barber salon, dentist and others.
  • Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Chief Operating Officer/Administrator.
  • Develop and implement policies and procedures for the identification of medically related social and emotional needs of the resident.
  • Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.
  • Provide information to resident/families regarding Medicare/Medi-Cal, and other financial assistance programs available to the resident.
  • Serves as an active member of the Quality Assessment & Assurance (QAA)/ Quality Assurance and Performance Improvement (QAPI) Committee and assists in carrying out the various tasks/activities/plans of action of the Committee.
  • Interview residents, or family members, as necessary, to obtain social history.
  • Ensure that social service progress notes are informative, descriptive of the services provided, capture the resident’s response, and completed at required frequencies.
  • Actively participate in specified routine meetings such as the daily stand-up, clinical meetings, care plan meetings, and Medicare meetings.
  • Lead/Direct the routine psychotropic medication review meeting/process.
  • Other duties as assigned.

Admissions:

  • Assists the Director of Community Relations with processing new admissions/re- admissions in the absence of the HCC Admissions/Medical Records Coordinator. This includes but is not limited to reviewing admission paperwork with the resident/family member, collecting signatures, and assisting to orient the resident/family member to the facility.

Care Plans, Assessments and MDS:

  • Schedules and coordinates Care Plan meetings with residents and their families and/or responsible parties.
  • Completes and update Care Plans as they relate to the social and psychological needs of the resident.
  • Encourage the resident/family to participate in the development and review of his/her plan of care.
  • Develop preliminary and comprehensive assessments of the social service needs of each resident.
  • Ensure that social services components of the MDS are completed and signed on a timely basis.

Minimum Qualifications

  • Associate’s degree from an accredited institution – Required.
  • Bachelor’s degree in social work, gerontology, psychology or another related human services field - Preferred.
  • Licensed Clinical Social Worker (LCSW)- Preferred.
  • 2 years of experience in skilled-nursing, acute or sub-acute setting – Required.
  • Two years’ experience in a social service capacity- Required.
  • Social Services Designee in Long Term Care Certification for Skilled Nursing – Preferred; if not already certified, must obtain within 6 months of hire.
  • Knowledgeable of Federal and State regulations related to skilled nursing social services or willing/open to learn.
  • Sensitivity and understanding of the issues related to aging services including, but not limited to resident rights.
  • Strong working knowledge of psychosocial, medical, and legal issues impacting the elderly.
  • Knowledge of relevant community resources.
  • Ability to relate/communicate appropriately and effectively to residents and their families, to other staff as part of an interdisciplinary team, and to personnel of other community agencies.
  • Strong case management, assessment and care planning skills.
  • Must be proficient with various software applications such as Microsoft Office, electronic medical records, and communication tools (e.g. Zoom).
  • Must demonstrate excellent skills in reading writing, speaking and understanding the English language. Other languages a plus.
