$95.00 – $100.00 per visit

Job Summary

Elysium Hospice Care is seeking a compassionate Social Worker (MSW) to join our interdisciplinary hospice team. The Social Worker provides emotional support, counseling, and resource coordination to patients and families facing terminal illness, ensuring dignity and compassion throughout the end-of-life journey.

Responsibilities

• Assess the psychosocial needs of patients and families related to illness and environment
• Provide emotional counseling, grief support, and crisis intervention as needed
• Assist with advance care planning and completion of directives
• Coordinate community resources and referrals for practical and emotional needs
• Participate in care planning and IDG (Interdisciplinary Group) meetings
• Maintain accurate and timely documentation according to hospice standards

Qualifications

• Master’s degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE) — required
• Minimum of one (1) year of healthcare experience — hospice experience required
• Demonstrates a strong understanding of hospice philosophy and end-of-life care principles
• Excellent verbal, written, and organizational skills
• Current CPR certification
• Possesses a valid driver’s license, auto insurance, and a reliable, insured vehicle in accordance with state and organizational requirements
• Must pass a criminal background check, drug screening, and physical exam prior to hire
