We’re seeking a dedicated and experienced Social Worker to lead and support our Social Services Department. Position Summary
The Social Services Director is responsible for planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services Department in accordance with federal, state, and local standards. This position ensures that residents’ medically related emotional and social needs are identified and met on an individual basis.
Key Responsibilities
- Plan, develop, and oversee social service programs in alignment with facility standards.
- Participate in discharge planning and resident assessments.
- Provide support and education to residents and families regarding community resources, Medicare/Medicaid, and financial assistance.
- Collaborate with medical staff and other departments to coordinate care and services.
- Maintain documentation, progress notes, and social service records.
- Assist residents and families in resolving emotional, social, and practical challenges.
- Lead and train Social Services staff, and participate in quality improvement initiatives.
- Ensure compliance with all regulations and facility policies.
Qualifications
- Bachelor’s degree in Social Work or a related human services field (sociology, psychology, rehabilitation counseling, etc.).
- Minimum 1 year of experience as a Social Worker in a healthcare setting (long-term care preferred).
- Licensed Social Worker (LBSW) or higher-level state licensure required.
- Strong interpersonal and communication skills.
- Knowledge of Medicare/Medicaid processes and long-term care regulations.
Work Environment
- Fast-paced healthcare setting with daily resident and family interaction.
- Must be able to handle emotional situations with professionalism and empathy.
- Occasional weekend or holiday work may be required.
Physical Requirements
- Ability to lift up to 25 lbs (occasionally up to 50 lbs).
- Must be able to move throughout the facility and assist residents as needed.

Pay: $61,590.33 - $76,581.90 per year
