The Social Worker is a member of the interdisciplinary team working in collaboration with the Physician. The Social Worker utilizes the care process consistent with their certification including initiation of interventions within individual scope of practice by providing concrete, non-clinical services. The Social Worker is responsible to oversee any necessary coordination of the interdisciplinary plan of care and provides for its consistency with the medical treatment plan. The patient care process includes:
- Assessment - an orderly process consisting of the identification, gathering and organization of subjective and objective data pertaining to the patient/family/significant other to make referral and implement a plan of social care and action.
- Counseling-professional application of social work methods and values in advising and providing guidance to individuals, families or groups for the purpose of enhancing, protecting or restoring the capacity for coping with the social environment.
- Consultation-discussion and evaluation focusing on data, goals and objectives including interventions with families, agencies, business, organizations, groups and communities for the purpose of problem solving.
- Planning - an organized step in the process that consists of the identification of actual or potential problems and desired outcomes (goals) that result in a documented plan of care. The plan of care provides direction for the patient/family/significant other and interdisciplinary team.. This step provides for documentation of required actions based upon findings identified during assessment and reassessment:
- Patients have problems identified and prioritized relative to biophysical, psychological, environmental, self care, educational and social status.
- Goals are identified consistent with the problem to provide direction for patient care and health promotion.
- Education is consistent with the identified problem and associated goals and is seen as critical to creating an environment that promotes patient empowerment.
EDUCATION: Bachelor's degree from an accredited school of social work required.
LICENSE/CERTIFICATION:
Valid driver's license required for the Social Workers charged to Case Management, Mission Health Services and Adult and Child Partial Programs. CSW (Certified Social Worker) certification required.
EXPERIENCE: Minimum 1 year experience, preferably within healthcare. Proficiency in service Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients, as defined on the unit specific Assessment and Evaluation Tool.
Potential for exposure to the hazards and risk of the hospital environment, including exposure to infectious disease, hazardous substances, and potential injury.
Position requires considerable walking and standing. Position requires frequent periods of driving a motor vehicle.
