Description:

We are seeking a Peer Support Specialist – Linkage to Care to engage with individuals struggling with substance use, mental health challenges, and social inequities. This role will focus on building trust, providing peer-to-peer support, and connecting individuals to critical health and recovery services. The ideal candidate will have lived experience with substance use recovery and a passion for supporting others in their journey toward wellness and self-sufficiency.

Key Responsibilities:

  • Conduct community outreach and engagement to identify individuals in need of support.
  • Provide peer-to-peer counseling and emotional support in a nonjudgmental, compassionate manner.
  • Assist individuals with navigating resources and connecting to care, including substance use treatment, mental health services, healthcare, housing, and employment assistance.
  • Work closely with ACCESS Harris County and other community partners to coordinate wraparound services.
  • Facilitate and support Naloxone (Narcan) training for individuals and community groups.
  • Maintain accurate records of interactions and referrals to ensure effective follow-up and program evaluation.
  • Participate in ongoing training and professional development related to harm reduction and peer support.
Requirements:
  • Lived experience with substance use recovery, mental health challenges, or related hardships.
  • Certified Peer Recovery Support Specialist (PRS, PSS, or equivalent) preferred (or willingness to obtain certification).
  • Experience in community outreach, harm reduction, or case management is a plus.
  • Strong interpersonal and communication skills with the ability to connect with diverse populations.
  • Knowledge of local resources and services available for individuals with SUD, mental health challenges, housing insecurity, or other barriers to care.
  • Commitment to harm reduction principles and a stigma-free approach to care.
  • Ability to travel within Harris County to conduct outreach and attend community events.
