Job Overview:

The Social Work Coordinator addresses needs that require the services of multiple professionals, agencies, healthcare facilities and human service programs. The goal is to ensure support coordination so that members with brain injuries receive the care and opportunities to which they are entitled. This position also is responsible for providing short term counseling in order to make certain of successful completion of the members goals. The Social Worker may also assist members in the work activities of the program, as well as, in the community. They will also facilitate weekly groups focused on adjustment and other relevant topics.

This position is with Opportunity Project. The Opportunity Project Clubhouse is a unique program created by and for adults with brain injuries who are seeking the next step in their journey towards recovery. We provide an environment of support, challenge and choices that creates independence and productivity in our members. Through the work environment in the Clubhouse, our members explore their strengths, abilities and interests so that they can establish and attain their individual goals. The ultimate goal is full integration back into family, work and community.

Qualifications:

Required:

  • Bachelor’s Degree in Social Work or related field
  • AHA BLS Provider Certification
  • Valid NJ drivers license and clean driving record
Preferred:
  • Master’s Degree
  • Experience working with individuals with disabilities
  • Bilingual in written and verbal communication
Certifications and Licenses Required:
  • AHA BLS Provider Certification
  • NJ Driver’s License
Scheduling Requirements:
  • Part Time
  • 20 hours/week
  • Weekend hours
Essential Functions:
  • Manages relationships with and delivers presentations to referral and funding sources
  • Works with the Program Manager to manage and maintence insurance authorizations as needed
  • Conducts intakes and tours for new referrals as well as gathers all medical, insurance and ID documentation
  • Participates in staff and case conferencing meetings
  • Works with caretakers to ensure members health, socialization, recreational and emotional needs are met
  • Coordinates and facilitates family meetings to discuss members progress, goals, challenges and incidents
  • Facilitates weekly adjustment groups for members
  • Writes progress notes as well as reports and letters to funding sources and relevant agencies
  • Regularly updates emergency contacts, healthcare providers and medication information
  • Reviews Member Manual, Photo or Video Approval and Release of Information Forms with members
  • Manages orientation process for new members
  • Assists members with securing funding, insurance authorizations, transportation, housing, SSI, SSDI, Medicaid, Medicare and commercial insurance
  • Provides one to one short term counseling and referrals
  • Generates monthly attendance reports for the billing department
  • Updates and shares community resources in the Clubhouse as well as through email and social media
  • Plans and coordinates workshops for families and caregivers on a variety of topics including maximizing entitlements, guardianship, housing, special needs trusts, Medicaid, Medicare and SSI, SSDI
