Job Overview

We are seeking a detail-oriented and organized Intake Coordinator to join our team. In this vital position, you’ll manage the patient intake and referral process, ensuring accuracy, timeliness, and compassionate communication with patients, families, and healthcare professionals. This role is ideal for someone who thrives in a fast-paced office environment, communicates clearly, and enjoys contributing to a mission-driven healthcare team.

Responsibilities

  • Receive and process incoming referrals and intake information by phone or email.
  • Complete referral/intake forms accurately and route them for admission approval.
  • Coordinate with clinical staff to schedule appropriately skilled personnel for new admissions.
  • Input scheduling and referral details into the computer system promptly and accurately.
  • Maintain patient confidentiality in accordance with HIPAA guidelines.
  • Communicate with patients, families, and referral sources in a professional and compassionate manner.
  • Assist with other administrative tasks as directed by the Administrator.

Qualifications

  • High school diploma or equivalent.
  • Proficient computer skills; ability to navigate scheduling and other systems.
  • Excellent written and verbal communication skills.
  • Strong attention to detail and organizational skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Familiarity with medical terminology and procedures.
  • Previous experience in a Home Health or healthcare administrative setting is preferred.
