Assignment Details:

• Location: Oakland, CA
• Practice Setting: Outpatient
• Patient Population: Adult
• Contract Length: Minimum 6 months, one-year preferred
• Shifts: Part-time 16hrs/week - 2x8 shifts Mon / Fri - Hours: 8:30am - 5pm
• Medication assessments and follow-ups with flexible appointment scheduling.
• Client allows 1.5 - 2 hours for initial assessments and 30-45 minutes for medication follow ups, depending on clinical complexity.
• Case management support in a multidisciplinary team setting.
• Ongoing symptom management
• Client has an E.H.R., Clinician's Gateway, so candidates must be computer literate
