Job Summary
We are seeking a dedicated and compassionate Social Worker with hospice and medical experience to join our team. The ideal candidate will be responsible for providing support and guidance to individuals and families facing various challenges, including end-of-life care. This role involves assessing client needs, developing treatment plans, and implementing interventions to promote well-being and stability. The Social Worker will work collaboratively with healthcare professionals, including physicians, nurses, and hospice staff, to ensure comprehensive care and support for clients.

Responsibilities

  • Conduct assessments to identify client needs and develop tailored intervention plans, including for patients in hospice care.
  • Provide crisis intervention and management services to individuals in distress, including end-of-life and family support situations.
  • Offer addiction counseling and support for clients struggling with substance use issues.
  • Facilitate access to behavioral health resources, hospice services, and other healthcare programs.
  • Mentor clients in developing coping strategies and life skills through motivational interviewing techniques.
  • Supervise case management staff and provide guidance on best practices in social work, including hospice-related protocols.
  • Collaborate with community organizations to enhance service delivery, including Section 8 housing assistance, child welfare programs, and hospice resources.
  • Advocate for clients’ rights and assist them in navigating Medicare, Medicaid, and other public assistance programs.
  • Participate in workforce development initiatives to empower clients towards self-sufficiency.
  • Support families in understanding hospice care, advance directives, and end-of-life decision-making.

Skills

  • Strong crisis intervention skills with the ability to remain calm under pressure.
  • Proficient in addiction counseling techniques and crisis management strategies.
  • Knowledge of behavioral health principles and practices related to social work.
  • Experience supervising staff or mentoring individuals in a professional setting.
  • Familiarity with childcare practices, particularly within the context of child welfare services.
  • Understanding of Section 8 housing regulations and resources available for clients.
  • Ability to engage clients effectively using motivational interviewing techniques.
  • Experience working in hospice care settings, including knowledge of patient care, family support, and collaboration with physicians and hospice teams.
  • Excellent communication, interpersonal, and organizational skills essential for successful case management.

Job Types: Full-time, Part-time, Per diem

Pay: $75.00 - $100.00 per hour
