Overview
We are seeking a dedicated and compassionate Care Coordinator that's an LPN as well, to join our healthcare team. The ideal candidate will play a vital role in ensuring that patients receive comprehensive care tailored to their individual needs. This position requires strong organizational skills, effective communication, and the ability to work collaboratively with healthcare professionals and families. The Care Coordinator will be responsible for managing patient care plans, facilitating communication among providers, and ensuring adherence to treatment protocols.
Duties
- Coordinate patient care across various settings including hospitals, nursing homes, assisted living facilities, and home care.
- Develop and implement individualized care plans based on patient assessments and medical histories.
- Serve as a liaison between patients, families, and healthcare providers to ensure seamless communication.
- Monitor patient progress and adjust care plans as necessary in collaboration with the healthcare team.
- Conduct intake assessments and maintain accurate medical documentation in compliance with HIPAA regulations.
- Utilize EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for effective patient record management.
- Provide education to patients and families regarding treatment options, medication administration, and health management strategies.
- Assist in triaging patients based on urgency of care needs.
- Ensure compliance with NCQA standards and other regulatory requirements.
Experience
Candidates should possess a variety of experiences that contribute to their ability to provide high-quality patient care. Preferred qualifications include:
- Critical care experience in settings such as ICU or emergency medicine.
- Medical office experience with familiarity in ICD-9/ICD-10 coding and medical terminology.
- Long-term care experience including geriatrics, memory care, Alzheimer’s care, or hospice care.
- Experience working with individuals with developmental disabilities or behavioral health challenges.
- Proficiency in patient assessment skills including vital signs monitoring, phlebotomy, medication administration, and basic life support.
- Familiarity with various medical specialties such as pediatrics, internal medicine, outpatient services, or surgical environments (OR experience).
- Knowledge of health coaching principles and community health resources.
