Position Title: LTSS Service Care Manager (Field-Based – Medicaid/Medicare)

Pay Rate: $33/hr

Job Summary

We are seeking an experienced Service Care Manager (Case Manager) to join our growing team. This role is field-based and will focus on coordinating long-term care services for members in Medicaid/Medicare programs, with an emphasis on geriatric patients and individuals with long-term care needs.

This is an excellent opportunity for a compassionate and detail-oriented professional who enjoys face-to-face field work and managing a high-volume caseload while making a direct impact on patient outcomes.

Key Responsibilities

• Evaluate member needs and recommend appropriate care plans and resources.
• Develop, coordinate, and monitor long-term care service plans.
• Collaborate with members, families, caregivers, and provider teams to ensure access to services.
• Provide resource support for members and families (employment, housing, independent living, etc.).
• Conduct field visits to assess member needs and ensure care plan adherence.
• Document and maintain case management activities in compliance with regulations.
• Support discharge planning and coordinate with healthcare teams to ensure continuity of care.
• Manage a high-volume caseload (50+ members) while delivering quality, cost-effective care.

Required Qualifications

• Bachelor’s degree in nursing, social work, psychology, or related healthcare field (or equivalent experience).
• 2+ years of case management experience (field-based strongly preferred).
• Experience with Medicaid/Medicare and long-term care populations.
• Proven ability to manage high caseloads (50+) effectively.
• Strong documentation and organizational skills.
• Proficiency with electronic health/medical records.
• Valid driver’s license and ability to travel locally (80–90% field work).
• Minimum typing speed of 40 WPM (typing test results required with application).

Preferred Qualifications

• Bilingual (Spanish/English) highly preferred.
• Experience with discharge planning.
• Familiarity with TruCare or similar care management systems.
