Job Description:
We are seeking a dedicated and compassionate Bilingual Care Coordinator fluent in Korean, Chinese, and English to join our team full-time. This role is essential in delivering a high standard of support and care coordination to our members and patients. You will work closely with Net Providers (NPs), patients, caregivers, and nursing staff to ensure a seamless and personalized customer service experience while advocating for the care and services our clients need.
This is position is a Hybrid position, 2-3 days at office.
Key Responsibilities:
- Establish and maintain strong, positive relationships with Net Providers and members.
- Coordinate and refer clients to appropriate community-based services and resources.
- Follow up with clients to ensure services have been accessed and needs are being met.
- Collaborate with internal teams and external partners, including attending meetings and participating in community or organizational events.
- Manage, organize, and maintain care coordination-related documents and records.
- Provide bilingual support and interpretation/translation services when necessary.
- Maintain confidentiality and adhere to HIPAA and all relevant privacy regulations.
- Identify service gaps and escalate complex cases to the appropriate department or supervisor as needed.
- Perform other related duties and assigned tasks to support the care coordination team and organizational goals.
Qualifications:
- 1–2 years of experience in care coordination, case management, social services, or a related healthcare field.
- Excellent interpersonal, communication, and customer service skills, with the ability to engage compassionately with patients and families from diverse backgrounds.
- Strong organizational and problem-solving abilities.
- Proficiency in using electronic health records (EHRs), customer service platforms, and other technology tools.
- Fluency in Korean, Mandarin, and English is required (speaking, reading, and writing).
- Bachelor’s degree in Community Health, Social Work, Public Health, or a related field is preferred.
Job Type: Full-time
Pay: $25.00 - $27.00 per hour
