Intake Coordinator /Case Manager (Bilingual Spanish/English)

Salary: $53,000-$55,000

Classification: Non-Exempt

Reports to: Care Connections Manager/ Case Management Senior Supervisor

Job Summary: The Intake Coordinator/Case Manager (IC/CM) is a member of the Care Connections Intake Team, working collaboratively to support client services. This position is responsible for screening all new potential clients to determine their eligibility for any one of our older adult services programs at Sunnyside Community Services (SCS). They will also maintain a small caseload of Adult Protective Services Clients (APS).

This position requires strong knowledge of all SCS programs and resources for older adults and their caregivers to appropriately connect them to the services that best match their needs. The IC/CM, when applicable, will also complete an intake for our Case Management program for eligible clients. As such, the IC/CM will work under the day to day supervision of the Care Connections Manager and will also be under the supervision of the Case Management Senior Supervisor for the APS caseload oversight. Good record keeping, computer knowledge and time management skills are essential.

Responsibilities:

· Assist with phone screening of clients by listening actively, assessing issues, and offering appropriate solutions or next steps

· Maintaining a friendly, patient, and helpful attitude, even with difficult callers

· Conduct intakes and make referrals to other available resources

· Identify and assist clients with basic information on Benefits and Entitlements

· Keeping calls efficient and managing a high call volume to maintain quality and client satisfaction

· Ability to multitask with handling calls while entering data, navigating systems, or referencing scripts and resources

· When necessary, contact emergency services when a client is in distress

· Conduct and record periodic communication with APS workers to determine the status of clients, in addition to discussing status of APS cases with Senior Case Management Supervisor

· Comfortable using Salesforce, VIVE, or similar CRM software

· Participate in trainings and conferences and agency meetings as required by NYC Aging

· Assist with outreach in the community

· Other duties as assigned by the Intake Manager

Qualifications:

· Experience in Human Service/Social Work field

· Excellent organizational and computer skills

· Good verbal and written communication skills

· Must be bi-lingual in Spanish and English

· Must have excellent interpersonal skills with an ability to establish rapport with staff, clients, informal supports and other community and government agencies

· Must have a minimum bachelor’s degree

Working Conditions:

This position currently operates on a hybrid work model with three days in the office and two days remote. Office setting.
