POSITION DESCRIPTION
Manage Case Management intake and supervise case aide/s working directly with older adults age 60 and older in order to identify client’s needs and service preferences, and work with client (and family, when appropriate) on services as needed.
RESPONSIBILITIES INCLUDE BUT NOT LIMITED TO
- Perform pre-screening and initial client intakes, including full assessment of needs and safety.
- Conduct 1 day/15 day/2-month monitoring calls.
- Provide information and referral to clients/potential clients and document accordingly.
- Consult with Assistant Program Director on new clients that should be assigned immediately based on needs/risk.
- Supervise case aides including signing off on case notes, providing weekly individual supervision, and support as needed.
- Assign intakes to Case Managers on Intake Support, Review intakes conducted by Case Managers, and create and maintain Intake Support Schedule.
- Train new Case Managers on conducting intakes.
- Collaborate with outside service providers to best address client’s needs.
- May carry a caseload.
- Prepare necessary reports regarding intake, caseloads, and waitlist.
- Maintain familiarity with all relevant DFTA standards including updates to standards and procedures.
- Other administrative and supervisory tasks as assigned by Assistant Program Director/Program Director.
- Participate in team meetings.
- Participate in DFTA and CASC training and education sessions.
- Participate in local education and informational sessions and conferences.
SUPERVISORY RESPONSIBILITIES
· Case Aide (s), Case Managers – as assigned by Assistant Program Director
EXPERIENCE AND QUALIFICATIONS
· BA/BS/BSW degree min., MSW/MS preferred
· Experience in providing direct services to the elderly preferred
· Supervisory experience (preferred).
· Fluent in a second language (preferred).
· Word, Excel essential.
· Assessment Skills: ability to thoroughly assess needs of older adults inquiring about services
· Detail Oriented: ensure entire intake is completed and documented in accordance with DFTA guidelines.
· Organized: ability to manage time, job responsibilities and prioritize tasks appropriately.
· Leadership: ability to identify strengths and weaknesses in staff and provide supervision, education and guidance to improve job performance, and to manage staff during crises and other stressful situations.
· Understands Diversity: accommodates linguistic and cultural differences; works well with colleagues and clients with diverse ethnic and cultural backgrounds and can guide staff in meeting these goals.
· Self-motivated: Able to work independently, works well under pressure and handle multiple priorities simultaneously.

SALARY
$61,000-$65,000/annually
Commensurate with experience
