Scope of Position: The MSW will work under the supervision of the Clinical Supervisor. The MSW will help in delivering clinical services to all residents. The MSW will work with the clinical team to conduct clinical interventions, and service delivery options and help clients get connected to community resources. Will construct Psych-socials, SPOA applications, and safety plans and conduct client assessments.
Essential Position Functions:
- Intake and assessment, orientation and screening process, documentation of all client related interactions into client database systems
- Assessing the needs and strengths of consumers to set service plan goals/ objectives, and conducting substance abuse, relapse prevention, and/or other clinically driven groups weekly.
- Ensure timely completion of documentation for every client contact, adhering to DHS and Weston policy and regulatory guidelines.
- Provide individual counseling sessions, and facilitate healthy recovery, independent living skills, and risk reduction groups.
- Provide outpatient and inpatient referrals and other community resources to the consumers.
- Carry a caseload of clients who are high-risk/high-need and act as a consultant assisting the clinical team in such cases. Work with the social service staff to identify high-risk clients and help connect these clients to additional supportive services.
- Co-facilitate groups if requested in areas including but not limited to wellness, leadership, housing, stability, employment and education, relationships, and healthy decision-making.
- Attend staff meetings, supervision, and trainings, as required. Obtain First Aid and overdose prevention Certification as well as Fire Safety Certification (F80 or F02)
- Assist in preparing the program for audits.
- Provide crisis intervention, as necessary.
- Compliance with requirements, including facilitating appropriate admissions and discharges, maintaining census, documentation and incident reporting
- Immediately reports serious incidents, serious incident allegations, incidents, or sensitive situations to supervisors. Completes incident reports in accordance with Weston policy.
- Ability to work with consumers/residents, families, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences.
- Communicate with Hospitals when clients are psychiatrically hospitalized and visit hospitals as needed.
- Perform duties of Case Managers and Operations staff as needed.
- Other duties as assigned by supervisors.
Educational Requirements: This position requires a Masters in Social Work degree or related social services degree. At least one year of experience working with clients who have mental health needs
Qualifications and Experience: Candidate must have at least one year experience working with adults who have a severe and persistent mental illness with or without a history of substance abuse. Candidate must have excellent time management skills; stress resistant, flexible; team player; open to feedback; creative; excellent organizational skills; willingness to perform CPR/First Aid/AED/CPI/Naloxone. Proficient in Microsoft Office applications including Word, Excel and knowledge of DHS CARES and AWARDS preferred.
Hours: 35 hours per week, variable schedule
