Position Summary

The Supportive Housing Case Manager provides intensive, person-centered services to individuals eligible for the Governor’s Opportunity for Supportive Housing (GOSH) program. This role supports clients discharging from or diverted from hospitals or other facilities in securing and maintaining stable, independent housing. The position integrates ODAT principles to deliver flexible, mobile, and responsive behavioral health and housing support.

Key Responsibilities

· Provide intensive tenancy support and housing navigation services to GOSH participants.

· Conduct in-person outreach and home visits to support housing stability and recovery goals.

· Collaborate with landlords, housing authorities, and community partners to secure and sustain housing placements.

· Develop and implement individualized service plans (GASP), including housing assessments, crisis plans, and subsidy applications.

· Utilize ODAT principles to deliver flexible, mobile, and client-driven services, including crisis intervention and behavioral health support.

· Maintain caseloads of approximately 1:15 to ensure high-quality, individualized care.

· Document services and submit reports (e.g., Monthly Activity Reports, GOSH Quarterly Reports) in compliance with DSHS standards.

· Participate in case staffing with ALTSA Supportive Housing Program Managers (SHPMs) and other service providers.

· Attend required trainings and contribute to continuous quality improvement efforts.

Qualifications

· Bachelor’s degree in Social Work, Psychology, Human Services, or related field (Master’s preferred).

· Minimum 2 years of experience in housing case management, behavioral health, or related field.

· Knowledge of Housing First, Harm Reduction, Trauma-Informed Care, and Motivational Interviewing.

· Familiarity with Washington State’s ALTSA, ProviderOne, and supportive housing systems.

· Ability to work independently, manage time effectively, and respond to client needs with flexibility.

· Valid driver’s license and reliable transportation required for field-based work.

Preferred Skills

· Experience with GOSH, FCS, or HARPS programs.

· Bilingual or multilingual abilities.

· Familiarity with Medicaid billing and documentation standards.

Job Type: Full-time

Pay: $24.00 - $29.00 per hour
