The High Intensity Care Coordinator plays a critical role in delivering intensive, community-based support to children and their families with complex behavioral health needs. This position is designed to ensure that consumers receive comprehensive, person-centered care that addresses both mental health and social determinants of health. This position requires a minimum of a Bachelor's degree in a Social Services field. Must have strong communication and interpersonal skills. Knowledge of Electronic Health Records (EHR) is preferred. Must have a valid driver license with clean driving record and proof of auto liability coverage, as well as dependable transportation.
Job Type: Full-time
Pay: $12.00 - $15.00 per hour
Expected hours: No less than 40 per week
