Care Coordinator – Full-Time (In-Office)

AccordCare Home Health, an accredited home health agency, is seeking an experienced Care Coordinator to join our Maitland, FL office. This pivotal in-office role ensures our team and patients are supported through efficient scheduling and coordination, specifically for Private Duty Home Care services.

Key Responsibilities

• Schedule and coordinate all field staff visits in accordance with the Plan of Care for private duty clients

• Work closely with the Clinical Manager to manage patient schedules

• Maintain accurate records in the computer system with schedule changes and visit authorizations

• Communicate with Intake, referral sources, sales staff, and team members to facilitate patient care

• Collaborate with the Billing Department as needed

• Facilitate payroll and participate in on-call rotation

• Address and resolve patient/family scheduling concerns

• Build and maintain strong relationships with staff, sales teams, and referral sources to ensure efficient service coordination

✅ Qualifications

• Minimum 1 year of Private Duty Home Care scheduling experience required

• Strong written and verbal communication skills

• Excellent customer service skills

• Ability to work effectively in a fast-paced, office-based environment

• Exceptional organizational and time management skills
