Position Overview
We are seeking a dedicated and empathetic Social Worker to join our palliative care monitoring program. The Social Worker will play a vital role in providing psychosocial support, conducting home visits, and ensuring patients and families have the resources they need to manage their care with dignity and comfort.
Responsibilities
- Conduct home visits to palliative care patients to provide emotional, social, and resource support.
- Collaborate with the interdisciplinary care team (nurses, physicians, and care coordinators) to support comprehensive care plans.
- Assess patient and family needs related to coping, mental health, caregiving, financial resources, and community supports.
- Provide counseling, education, and advocacy for patients and families.
- Facilitate access to community resources, benefits, and supportive services.
- Document assessments, interventions, and progress in the care management system in compliance with standards.
- Serve as a liaison between patients, families, and healthcare providers.
Qualifications
- Master’s Degree in Social Work (MSW) required; LCSW or equivalent state licensure strongly preferred.
- Experience in palliative care, hospice, geriatric care, or home health strongly preferred.
- Excellent communication and interpersonal skills with a compassionate and empathetic approach.
- Strong organizational and documentation skills.
- Ability to work independently and collaboratively as part of a care team.
- Reliable transportation and willingness to travel locally for home visits.

Job Types: Full-time, Part-time, Contract
Pay: $49,129.02 - $80,000.00 per year
Expected hours: No more than 40 per week
