BayCare Health Systems is currently seeking a Discharge Planner 1 to join our outstanding and compassionate team at Behavioral Health Center !

• Facility: Behavioral Health Center
• Location: Tampa, FL
• Status: Full time, Hourly
• Shift Hours: 8:00 am- 4:30 pm
• Weekend Work: Occasional
• On Call: No
  
  

The Discharge Planner I Responsibilities Include:

• Provides assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting.
• Possesses a clear understanding of discharge and transition planning and linkage with community resources.
• Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations.
• Provides support as active member of the treatment team.
• Will perform other duties as assigned by supervisor.

Required

Certification:

• BLS (Basic Life Support)
  
  

Experience:

• Behavioral Health - 1 year
• Child and Adolescent - 2 years (SIPP Only)

• Bachelors - Counseling/Health Education/Related Field/Social Work/Human Services/Psychology/Nursing
