Pay Range: $38 - $40/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Remote – Making face to face visits at members home or other agreed upon public locale. Flagler County primarily, Alachua, Citrus, Lake, and Marion
• Position is autonomous with Care Manager to member face to face visits
• COORDINATING CARE FOR SPECIAL NEEDS FAMILY ON THE CMS PLAN
• Set tasking of assigned caseload Max 200 members
• Virtual with a couple of in person meetings per year
• Remote/Autonomous high tech work environment with field visits and telephone contact of members

Key Requirements and Technology Experience:

• Key skills; LCSW, LMHC, Pediatric
• Required: LCSW/LMHC license active in Florida
• Must haves: Pediatrics – Minimum of 2 years
• Bilingual preferred/highly encouraged,
• Experience with electronic medical records, Basic Excel skills
• Performance indicators: Caseload compliance
