  • Location: Remote, must be able to travel to IL for occasional onsite meetings (typically 1-2 times per year)
  • Schedule: Monday-Friday 8:30a-5p CST (no weekends or holidays)
  • Major Responsibilities:
  • Care Coordination of health care services (DME, transportation, etc.) for patients in the Advanced Care at Home (ACAH) program. Responds to clinical inquiry calls regarding Advanced Care at Home, triage as needed to care team and escalates potential risk management situation to supervisor. Assist with patient virtual monitoring as needed.
  • Practices and creates an environment that supports and sustains effective communication with internal and external customers. Interfaces communication between Advanced Practice Clinician (APC) and other service providers. This includes case management, home health, hospice, primary care physician, physician specialists and hospital staff. Provides basic case management to review symptom control with the patient, prescription refills, scheduling/retrieving results on diagnostics, labs, consultant visits and procedures.
  • Completion of referral process for appropriate Advocate Aurora Health departments. Ensures data entry into the system is complete for the referral, insurance requirements, and medical necessity support through researching medical history. Acts as a clinical resource to coordinate complex cases for safe and appropriate transition to other care settings.
  • Assists with patient outreach under clinical supervision. Perform regular outreach calls to provide patient and family support and education. Provides disease management instruction and education to patients and their families. Obtains necessary medical information regarding the patient's health status, current medications and goals of care from appropriate sources.
  • Effective caseload patient management through generating and monitoring reports.
Licensure, Registration, and/or Certification Required:
  • Licensed Practical Nurse (LPN) license issued by the state of Illinois
Education Required:
  • Completion of an accredited or approved program in Practical Nursing.
Experience Required:
  • Typically requires 3 years of experience in a LPN role.
Knowledge, Skills & Abilities Required:
  • Excellent telephone, interpersonal and written communication skills.
  • Knowledge of Medicare rules and regulations.
  • Strong commitment to teamwork and customer service both internally and externally.
  • Working knowledge of managed care environment.
  • Ability to work in a fast-paced environment with established time constraints.
  • May be required to travel to various locations to attend meetings or provide care coordination support to team.
  • Good Documentation Skills.
  • Computer proficiency including the ability to utilize software programs used by the organization.
  • Able to perform and prioritize multiple functions or tasks.
  • Able to effectively deal with multiple changes.
Physical Requirements and Working Conditions:
  • Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.
  • Must be able to:
  • lift up to 50 lbs. from floor to waist.
  • lift up to 20 lbs. over the head.
  • carry up to 40 lbs. a reasonable distance.
  • Must be able to:
  • push/pull with 30 lbs. of force.
  • perform a sliding transfer of 150 lbs. with a second person present.
  • Position requires use of foot pedals on carts or machines.
  • Must have functional vision, speech, and hearing.
  • May be exposed to mechanical, electrical, chemical, and radiation hazards as well as blood and body fluids; therefore, personal protective equipment must be worn as necessary.
  • Operates all equipment necessary to perform the job.
