Position Title: Clinical Social Services Lead

Location: San Jose, California

Salary Range: $107,000 6 $142,000 USD

Overview

We are seeking a skilled and compassionate leader to direct our social services and behavioral health team. This role focuses on program quality, regulatory compliance, team development, and partnership with internal and community stakeholders.

Key Responsibilities

  • Lead, mentor, and develop social work and behavioral health staff (recruiting, onboarding, coaching, performance management).
  • Oversee client assessments and individualized care plans; participate in interdisciplinary care meetings.
  • Ensure timely implementation and monitoring of interventions aligned with client needs.
  • Maintain compliance with documentation standards, client rights, and grievance procedures.
  • Manage behavioral health programs (counseling, support groups, education) and coordinate with community providers.
  • Support end-of-life care planning and coordination.
  • Conduct site and home visits to evaluate service delivery and client needs.

Qualifications

  • Master's degree in Social Work (MSW required); licensure preferred.
  • 2 to 5 years of social work experience with at least 2 years of supervisory/leadership experience strongly preferred.
  • At least 1 year of experience working with older adults or medically fragile populations.
  • Strong clinical judgment, organizational skills, and clear communication abilities.
  • Valid drivers license, clean driving record, and proof of insurance.
