About the Role:

The Intake Coordinator plays a critical role in the health care services sector by serving as the primary point of contact for new patients entering the system. This position ensures that all patient information is accurately collected, verified, and entered into the health care database to facilitate seamless care coordination. The Intake Coordinator collaborates closely with clinical staff, administrative teams, and external providers to schedule appointments and manage referrals efficiently. By maintaining clear communication and thorough documentation, the Intake Coordinator helps optimize patient flow and enhances the overall patient experience. Ultimately, this role supports the delivery of high-quality health care services by ensuring that the intake process is smooth, compliant, and patient-centered.


As a Intake Referral Coordinator, you will:
  • Receives and appropriately documents new referrals in a timely manner.

  • Verifies each patient’s payor source and maintains and tracks insurance authorizations and initial certifications.

  • Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group.

  • Provides effective communication to patients, staff members, other health care professionals and referral sources.

  • Responds to Agency and patient needs in a professional and creative manner.

  • Demonstrates commitment, professional growth and competency.

  • Participates in Agency-sponsored in-service training.

  • Participates in the Agency’s QAPI program.

  • Promotes the Agency philosophy and administrative policies to ensure quality of care.


Requirements
  • Minimum one year of experience in referral intake is preferred

  • Experience with third party payers, Medicare and Medicaid highly desirable

  • Medical Field background preferred, including familiarity with medical terminology

  • Excellent computer and customer service skills

  • LVN/LPN preferred

  • Valid Driver’s License

  • Valid Auto Insurance

  • HomeCare HomeBase experience preferred
