The Transitional Housing Case Manager provides direct support to youth living in transitional housing. This role helps young adults set and reach goals around housing, employment, wellness, and stability. The Case Manager is the primary therapeutic presence in the housing program, providing transportation, resources, and daily support to residents.

Qualifications
QUALIFICATIONS & REQUIREMENTS
- Bachelor’s degree in Social Work or related field; lived experience valued.
- 2 years of direct youth services experience.
- Experience in housing navigation or transitional housing preferred.
- Strong documentation, relational, and problem-solving skills.
- Valid Drivers License and reliable transportation.
- Frequent walking, standing, kneeling, crouching, lifting up to 35 lbs.
- Site visits
- Indoor/outdoor work; exposure to weather and street-based environments.
- Attention to detail and ability to manage multiple tasks.
- Champion YMCA core values and model trauma-informed care.
- Manage a caseload of residents; develop and implement service plans.
- Provide weekly check-ins, goal tracking, and housing navigation.
- Support residents with transportation, life skills, and crisis intervention.
- Partner with the Housing Manager on resident behavior concerns and compliance.
- Document services in HMIS and maintain accurate files.
- Collaborate with community resources for referrals and long-term planning.
