We are seeking highly motivated individuals to join our team! In this role you will be helping us educate our consumers on available services across multiple state locations.
Responsibilities:
- Ensure our patients have dedicated home services.
- Manage in bound and outbound contacts under defined follow up timelines
- Educate and inform consumers about the range of care services available in different state locations
- Guide clients through the enrollment process, ensuring all necessary paperwork and requirements are met
- Maintain accurate and up-to-date records of client information and interactions.
- Provide outstanding customer service by addressing inquiries and concerns promptly and professionally, while advocating for our patients and caregivers through the process to start home care services
Requirements:
- Previous experience in home care intake coordination is preferred
- Excellent interpersonal and communication skills
- Proficient in Microsoft Office and Excel
- Excellent organizational skills, attention to detail, and the ability to prioritize tasks to ensure efficient workflow
- A proactive go-getter attitude that enables you to find creative solutions and make the most of available resources
- Bilingual in Spanish or Haitian Creole a plus!
