Social work also sensitizes other health care providers about the social psychological aspects of illness. *The Licensed Clinical Social Worker (LCSW) provides clinical coordination and therapeutic interventions through assessment, coordination, referral and Interdisciplinary planning for high-risk population. The Licensed Clinical Social Worker also links people to systems that provide them with resources, services and opportunities to achieve and maintain maximum functioning. Additionally, provides counseling and support to patients/families regarding adjustments to the disease process and illnesses.
