Job Summary
We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that our clients receive the highest quality of care tailored to their individual needs. This position involves coordinating care services, communicating with families, scheduling, and managing direct care staff.

Duties

• Assess client needs and develop personalized care plans that align with their requirements.
• Coordinate and oversee the delivery of care services, ensuring compliance with HIPAA regulations.
• Communicate effectively with clients, families, and caregivers to provide updates and address concerns.
• Administer medications as prescribed and monitor clients for any changes in condition.
• Maintain accurate records of client care, including medical terminology and documentation of services provided.
• Assist clients with daily living activities while promoting independence and dignity.
• Ensure a clean and safe environment for clients by adhering to health and safety standards.

Experience

• Previous experience in caregiving or related fields is preferred, particularly in assisted living.
• Familiarity with medical terminology and medication administration is highly desirable.
• A background in nursing or healthcare support will be beneficial but is not mandatory.
• Strong organizational skills and attention to detail are essential for effective coordination of care services.
• Excellent communication skills to interact compassionately with clients and their families.
