Position Description: Community Clinic is seeing a Care Coordinator to join our team at our Rogers Medical Clinic! The Care Coordinator supports patients and care teams by closing preventive and chronic care gaps, coordinating referrals and care transitions, addressing social drivers of health and improving access to services. The role advances FQHC priorities (UDS/PCMH/eCQMs) through proactive panel management, patient outreach and closed-loop coordination – within scope of license and under appropriate supervision. This position is fully in-person and primarily located in Rogers, AR but will occasionally travel to our Siloam Springs location. Schedule is Monday-Friday 7:00a – 4:00p or 8:00a-5:00p.
Essential Job Functions
- Provides care coordination review, planning and follow-up services to assure patients progress through the continuum of care.
- Use registries/reports to identify patients due to screenings, vaccines, chronic disease monitoring, and follow-ups (UDS/HEDIS/eCQMs).
- Manages care transitions after ER and inpatient hospital discharge, to facilitate timely engagement with Primary Care Provider.
- Verifies HIPAA documentation at every point of contact.
- Provides patient outreach to ensure compliance with healthcare programs.
- UDS/HEDIS care gaps, PRAPARE SDoH screening, enabling services documentation, closed-loop referrals, transitions of care.
- Facilitates preventive wellness services and improvement in patient health outcomes, utilizing population health tools and reports.
- Provides patients with information on health promotion, chronic disease management, healthcare services and resources to support their needs and goals.
- Coordinates the integration of social service, behavioral health, and case management into patient care.
- Adheres to departmental goals, objectives, standards of performance, and policies and procedures.
- Ensures compliance with quality patient care and regulatory compliance.
- Collaborates with clinical and other case management staff in the development and execution of the plan of care and achievements of goals.
- Enhances a collaborative relationship to maximize the patient’s and family’s ability to make informed decisions.
- Communicates with care teams at regular intervals and develops an effective working relationship with care teams and patients.
- Promote effective and efficient utilization of clinical resources. Supports practice mission and goals.
Knowledge and Critical Skills
- Knowledge of clinical practices in an ambulatory care, family practice setting.
- Knowledge of risk management and QI/QA principles, practices and management.
- Knowledge of HIPAA/HITECH compliance.
- Ability to work and function independently and within a team.
- Strong interpersonal skills and the ability to work effectively with people of all backgrounds.
- Knowledge of computer software such as Microsoft Office Software.
- Able to perform proficiently on current practice management system and/or electronic medical records.
- Engages in professional development activities, such as trainings and CEUs. Adept in Motivational interviewing
Qualifications
- High School Diploma or equivalent required
- Registered Medical Assistant required
- 2 years RMA experience in Primary Care or Pediatrics. required Bilingual (Spanish/English) preferred
